All product requests for trial or permanent use must be approved by DUHS Supply Chain / Service Line Council prior to introduction in the clinical setting.

Please follow these steps when requesting a product review:

  1. Review product request with Service Line Leadership and confirm support.
  2. Enter your request via the system link below, including completing the conflict of interest questions. If you are not a current user and do not have access to Lumere, please request access through the DUHS Clinical Value Analysis Team by calling 919-681-5900.
  3. Once your request has been reviewed and qualified to advance to project status, you will be notified and scheduled to present your request through one of the Health System Councils. Once confirmed for a Council agenda, you will be asked to complete a brief overview presentation for your item request.
