Clinical Engineering, Research Equipment and Medical Surplus Property Disposal Requests
Duke North Hospital, Duke Regional Hospital, Duke Raleigh Hospital
Large, time-sensitive requests:
- Enter a one-line request for "medical equipment" into the online asset disposition tool.
- Call Storr to schedule a pick up at 919) 313-3890 or email dukerequests@storr.com.
- Storr will contact the Surplus Property office for a copy of the one-line manifest.
- Storr will arrive at the pick-up location and itemize each item on a manual pick-up form.
- Department will complete and affix orange labels to each item (provided by Storr).
- Clinical Engineering will initial the manual pick up form to verify there is no data on the items.
- Clinical Engineering will remove all Clinical Engineering tags (not Duke asset tags) prior to shipment.
- Storr will then remove all items from the pick-up location.
- Storr will then convert the manual pick up form to an Excel spreadsheet in the format provided and forward the spreadsheet to the Surplus Property Office within two business days.
- Once cleared by Clinical Engineering, enter item into online asset disposition tool.
- Complete Surplus Medical / Research Equipment Label including condition information and affix to item. This label will be provided by Clinical Engineering or moving services provider. **Please be sure to verify condition for donation purposes prior to pick up!
- Clinical Engineering will coordinate pick up with Storr.
- Item will be collected and delivered to staging area until it is matched with a GH PLUS approved project.
PDCs, Clinics and Labs
- PDCs, Clinics and Labs call Clinical Engineering at (919) 681-2525 to request the removal of surplus items.
- Requester should have the Clinical Engineering device tag number available at which time items will be cleared for pick up.
- Clinics remove the Clinical Engineering device tags ONLY (not Duke asset tags).
- Once cleared, PDCs, Clinics and Labs enter item into online asset disposition tool.
- Complete Surplus Medical / Research Equipment Label including condition information and affix to item. This label will be provided by Clinical Engineering or moving services provider. **Please be sure to verify condition for donation purposes prior to pick up!
- PDCs, Clinics and Labs will be contacted by Storr to schedule the pick up, at which time a cost center charge code for billing purposes is required.
Research Equipment - Lab Equipment
- Enter item into online asset disposition tool.
- Complete Surplus Medical / Research Equipment Label including condition information and affix to item. This label will be provided by Storr at the time of collection. **Please be sure to verify condition for donation purposes prior to pick up!
- Requester should have the department's cost center charge code available when making the pick up request.
- Complete Laboratory Statement of Hazard Assessment for any equipment which has a Bio-Hazard or Radioactive label/sticker and attach to equipment. If a radioactive label/sticker is present, contact OESO Radiation Safety Division (919) 668-3188 to have the equipment cleared prior to pick up.
- Storr will coordinate a collection date and time.
- Item will be collected and delivered to staging area until it is matched with an approved project.
